Terms of Use and Accommodation Agreement

Accommodation Agreement

I authorize the Office of Accessibility and Accommodation (OAA) staff to discuss my accommodation needs with University Departments. This consent shall remain in effect until revoked by me, in writing. I agree to follow the guidelines for accommodations and services indicated in the OAA Student Handbook which I have been given access to via the Student Handbook webpage. I will notify my accommodation coordinator if my disability status changes, and I understand that requesting accommodations other than those indicated may require additional documentation.

If requesting any of the accommodations listed below, carefully review the corresponding agreement:

  • Testing Accommodations
    • Tests Scheduled in the OAA Test Center:
      • Only tests that are administered in person on campus should be scheduled in the OAA Test Center. The OAA Test Center is located in Fretwell 230
    • If eligible for testing accommodations in the OAA Test Center, I will follow the CODE OF STUDENT RESPONSIBILITY (U.P. 406), and the CODE OF STUDENT ACADEMIC INTEGRITY (U.P. 407) at all times. I understand that personal items are not allowed in the testing rooms, that these rooms are monitored by video surveillance, and that computer activity in the Test Center is monitored. To ensure test integrity, the use of cell phones and smartwatches is prohibited during testing. The only exception is for Duo authentication on online tests, if required. Students may not access their cell phones during breaks. Additionally, students found in violation of the cell phone policy will be reported to their instructor(s).
    • Online Testing: For tests administered online by the instructor that students can take anywhere (at home, in the Library, etc.), these tests will not be scheduled in the OAA Test Center. Students should contact the instructor prior to the test date to be sure that applicable accommodations are in place. It takes time for instructors to set up accommodations in Canvas. Students should notify their instructor(s) and their assigned accommodation coordinator before beginning the test if accommodations are not in place at test time.

Scheduling Tests: Students are responsible for scheduling tests and finals via the Student Portal by the deadlines. If deadlines are missed, the Office of Accessibility and Accommodation cannot guarantee space.

* TEST DEADLINES:

  • Tests, Mid-terms or Quizzes:
    • Fall/Spring Semesters – Schedule at least 6 days before the test.
    • Summer Semesters – Schedule at least 3 DAYS before the test.
  • Final Exams: Finals Deadlines differ by semester and are posted. Visit the Test Scheduling section for our specific Finals deadline.
    • During Finals: The high volume of students produces limited space in FRET 230. We utilize additional classroom space outside of our office for final exams. Headphones and ear plugs are available to help reduce distraction. We are unable to guarantee space in FRET 230 or individual rooms during the final exam period as first priority will be given to testers with assistive technology.​
  • Online Testing: Remember for tests administered online by the instructor that students can take anywhere, students should contact the instructor prior to the test date to be sure that applicable accommodations are in place. It takes time for instructors to set up accommodations in Canvas. Notify instructor(s) and the assigned Accommodation Coordinator before beginning the test if accommodations are not in place at test time.

  • Supplemental Class Notes: To activate this service, first I must request “supplemental class notes” on my Letter(s), and next I must request a Note Taker using the “Request a Note Taker” module in the Student Portal. Additionally, I will attend class, take my own notes, and notify the Office of Accessibility and Accommodation of any issues related to the note taking service in a timely manner. I understand that I am not permitted to share these notes with other students in any form and that doing so is a violation of academic integrity.
  • Per Course Attendance Adjustment Agreement: Reasonable adjustment for absences may be appropriate for students with chronic conditions experiencing unpredictable, brief flare-ups resulting in interruptions that can interfere with course activities. This accommodation is intended to provide limited flexibility to the course syllabus attendance policy. The Office of Accessibility and Accommodation (OAA) will directly facilitate creating and documenting a course-specific plan. Upon notification from Office of Accessibility and Accommodation, Instructors will be prompted to complete the Per Course Attendance Adjustment form with information about their course policies. Office of Accessibility and Accommodation will review that information and issue a course-specific agreement for the student and instructor that will outline the number of allowable absences. Students are responsible for following the agreement and for notifying their Instructor and Accommodation Coordinator about any disability-related absence within 48 hours of it occurring. The nature and structure of the course guides what is reasonable.
  • Equipment/Classroom Aid Loan: Office of Accessibility and Accommodation provides equipment and/or classroom aids for short term loan to UNC Charlotte students registered with our office. Such equipment or classroom aids may be loaned to students in accordance with their accommodations and for academic purposes only. I agree to abide by the following terms and conditions: (1) I am personally responsible for all equipment or classroom aids that I check out, and will return all loaned items within the specified date. (2) I am responsible for returning all loaned items to the Office of Accessibility and Accommodation. (3) Failure to return loaned items at the end of the loan period will result in a hold on my academic record. (4) Missing, stolen, or non-functioning items must be reported immediately to Office of Accessibility and Accommodation. (5) I am personally responsible for any damaged and/or missing items loaned to me and will be charged for missing or damaged items loaned to me. I understand that I am personally responsible for repaying the Office of Accessibility and Accommodation the full replacement cost of the loaned item(s).
  • Accessible Texbooks (e-text): I agree that I will not copy, reproduce, modify, or distribute the alternate format materials nor allow anyone else to do so pursuant to the requirements of the Copyright Act of 1976 as amended (17 U.S.C. 101 et seq.). Failure to abide by this is a violation of federal copyright laws and the CODE OF STUDENT RESPONSIBILITY (U.P. 406), and the CODE OF STUDENT ACADEMIC INTEGRITY (U.P. 407) and may result in disciplinary action.
  • Audio-Record Class Lectures: Permission to audio-record lectures provides access to course content for students who have disabilities that limit their ability to take notes effectively. Please note: This accommodation is strictly limited to audio-only recording. Audio recordings are for personal study use only, and any sharing or distribution of such recordings is prohibited. Smartpens, Genio Notes App, or digitally-recorded audio lectures may not be used against the instructor, TAs, guest lecturers, or students whose classroom comments are recorded as part of class activity. Information contained in the recorded lecture is generally the intellectual property of the creator, protected under federal copyright laws, and may not be copied, broadcast, published, or quoted without the explicit consent of the speaker. This prohibition includes placing audio recordings or transcripts of recordings on any website or the Internet. Furthermore, the student agrees not to record or release private information shared by classmates and agrees to permanently delete all audio recordings once they are no longer needed for academic work.
  • PCA in Class: Students who require the use of a Personal Care Attendant (PCA) in class to fully participate in the university experience must work with the Office of Accessibility and Accommodation to complete all necessary forms and provide all necessary paperwork for ensuring their PCA(s) has approved access to campus. Additionally, the PCA is employed by the student, not the University. While present, the attendant’s role is to perform necessary duties without taking an active role in the course or interfering with classroom activities, discussions, or academic instruction. Furthermore, the PCA must not occupy seating or spaces reserved for enrolled students, and their conduct must align with all applicable policies, specifically University Policy 406 (Code of Student Responsibility) and University Policy 407 (Code of Student Academic Integrity).
  • PowerPoints:  Access to PowerPoints that are used in class are provided by instructors as an accommodation and are for your personal study use only. PowerPoints are often copyrighted materials. PowerPoints provided as a reasonable accommodation may not be shared with others, posted online, or distributed in any manner.
  • Per Assignment Extensions: This accommodation supports students with chronic conditions who experience unpredictable, brief increases in disability-related symptoms. This accommodation applies on a case-by-case basis, is not automatic for all coursework, and cannot be applied retroactively. When an extension is needed, students must contact both their instructor and Accommodation Coordinator prior to the deadline whenever possible, or within 24 hours of the deadline. The instructor determines the reasonable extension length depending on the nature of the assignment and overall course progression and communicates the approved due date to both the student and OAA. If an extension cannot be granted, your instructor should contact OAA to review the request. Standard late penalties may apply if agreed-upon extension deadlines are missed or if late submissions are unrelated to a disability.

With my acknowledgement of this agreement, I also understand the following:

  • I understand that I need to meet with my instructor(s) to discuss my accommodations.  Connect with instructors by email or meet during their office hours. This necessary step helps ensure that you and your instructors are on the same page regarding your accommodations and logistics.
  • I understand that I may choose not to use any of my approved accommodations. By not checking approved accommodations, I’ve declined those accommodations and they will not appear on my Accommodation Letters.
  • I understand that it is my responsibility to address any concerns or changes regarding my accommodations or disability needs in a timely manner by notifying Office of Accessibility and Accommodation that I need to connect with my assigned Accommodation Coordinator. If the situation is complex, I will schedule an Advisement appointment with my Accommodation Coordinator to get further assistance.
  • By requesting accommodations via the Accommodation Student Portal, I understand that I have agreed to abide by the terms outlined in this agreement.
